Join Our Atomic HHC Team - HHA (PCA or CNA) - NYE COUNTY PAHRUMP

If you're interested in in applying. Please note the following requirements:

· Location: You must reside in Pahrump or within a 20–30-minute commute.

· Shifts: Availability for 12-hour day or night shifts is essential.

· Standby Shifts: Must be available for standby paid shifts, where you receive a flat fee for being on standby for a designated shift. If called in, you will earn your daily fee plus an hourly rate for hours worked.

· Background Check: You must pass a background check.

· Certification: If you are not a CNA in Nevada, you need to have a PCA certificate or be willing to take a 1 or 2-day PCA certification, which our company will pay for.

· Compliance: You must complete all state-mandated requirements. Our company covers the cost of TB tests and physical exams. If you have one within the past 6-8 month that may be used. 

· CPR Certification: You need to be CPR certified.

· Availability: Applicants should be able to meet all requirements within 10 working days of application acceptance. We are seeking individuals who can work between 12 to 36 hours per week.

 Atomic Home Health Care is looking for a motivated and compassionate Case Manager to coordinate care for patients receiving our specialized Home Health Care Services, including those covered under DEEOIC, FECA, and Black Lung benefits. This role is essential in ensuring patients receive comprehensive, high-quality, and patient-centered care.

Responsibilities:

  • Coordinate and oversee individualized care plans for home health patients
     
  • Collaborate with physicians, nurses, and other health professionals to ensure continuity of care
     
  • Serve as the primary contact for patients and families regarding care needs and services
     
  • Monitor progress, document outcomes, and adjust care plans as necessary
     
  • Ensure compliance with state and federal regulations
     

Qualifications:

  • Current RN license in Nevada (preferred) OR equivalent clinical background in case management
     
  • Prior experience in home health care or case management strongly preferred
     
  • Strong communication, organizational, and problem-solving skills
     
  • Knowledge of DEEOIC, FECA, or Black Lung programs is a plus
     
  • Reliable transportation, valid driver’s license and able to pass a background check.
